Assessing Your Depression, Anxiety and Stress Depression, Anxiety & Stress No Login This scientific questionnaire was designed to help give you a quick snapshot of your levels of depression, anxiety and stress. Not surprisingly it's called the Depression, Anxiety, and Stress Scale, and it's a research tool that has been validated over decades of scientific studies (Lovibond & Lovibond 1995). It has just 21 questions and takes about 3 minutes. Please read each statement and select the response that indicates how much it applied to you over the past week. There are no right or wrong answers. Don't spend too much time on any statement. To indicate how much each statement applied to you over the past week, use the following rating scale: 0: Did not apply to me at all 1: Applied to me to some degree, or some of the time 2: Applied to me to a considerable degree, or a good part of time 3: Applied to me very much, or most of the time Keep in mind that this assessment doesn't provide a clinical diagnosis.
